Requirement Before you can start a Travel Assignment and before you can start working Per Diem. All Required documents are as follows:

Health Statement/Physical (within current year)
2 Step TB, TSPOT, Quantiferon Gold (or documentation of 2 TB screenings within current year)
Immunization Records (MMR, HEP B, Tdap, Influenza and Varicella vaccinations or positive titers)

Candidates that can include copies of the following documents when applying will be considered for the assignment right away and can be submitted for an interview ahead of the competition to secure the assignment before it is filled:

Application and updated copy of resume (completed online during application process, please email MS Word copy of resume)
Credentials for BLS, ACLS and any other required for specialty (front and back copy with signature)
Online skills checklists (completed online during application process)
2 Professional References from pervious or current supervisors (please make sure the references are from managers or supervisors and cover a year of employment within the speicalty you are applying for)
Driver License and Social Security or (2 forms of Government ID,ex:Passport)
License/Certification for the job applying for (copy of front and back)
Diploma (from highest education achieved)
